Our Care at Home and Housing Support/Domiciliary Care services assist people to live independently in their own homes and communities and to have choice and control over their daily lives.

We offer these services to a wide range of people, including older people, people with mental health problems, physical and learning disabilities, HIV-related difficulties, palliative care needs and others who need support.

Momentum Care also provides support to people following hospital discharge and those who require short-term support and re-enable them to live as independently as possible.

Working closely with individuals and their families, we develop a personalised plan of support which is tailored to meet the specific needs of each person. We offer flexible support, ranging from a few hours per week to 24-hour support.
