Careconnect provides community-based care services, including full 24-hour support, to a wide range of people including those with physical disabilities, the frail elderly, people with HIV related difficulties, those who have sustained head injuries and terminal cancer patients who wish to remain in their own home.
Services available:
- Care and Support - bathing/showering, dressing, continence care, rising and tuck-in services
- Daily Living Services - housework, meal preparation, befriending, shopping, pension collection
- Transport Services - to hospital appointments, when visiting friends, shopping
- Short Breaks - supporting people to take a planned break from their local environment
