Community Solutions is an individually tailored package of support for adults with brain injury, which helps them to live independently in their own homes, within their own community.
Momentum's combined expertise in the fields of brain injury and care allow a specialised service to be offered, ranging from a few hours to 24-hour care.
Community Solutions encourages people to increase their personal independence and take part in their local community by accessing local resources.
Staff are recruited in consultation with the service user and family to ensure their specific needs are met.
Through Community Solutions, service users can learn the tools and techniques that empower them to make informed decisions towards leading meaningful and dignified lives.
Key elements of Community Solutions include support with daily living skills, communication, emotional and behavioural support and social skills training.
Psychological support and Information and Assistive Technology support can also be accessed through the service.
This service is available in: Glasgow, Ayrshire and Aberdeen.
