Momentum Care assists people with wide ranging needs to participate in community-based  activities and short breaks.

We support individuals to realise their own potential and to achieve their goals by accessing activities of their own choice. Momentum Care has supported people to participate in a wide range of activities including sports, music lessons, outdoor pursuits and driving lessons.

The service is tailored to suit the needs, preferences and aspirations of the individual. Choice is further promoted through accessing a respite short break to a destination of the supported individuals choice.

Working with individuals and their families/carers, Momentum Care ensures people have full choice and control over the service they receive and that they are given the right support to achieve their goals.
